I have the combo. We make more progress when my daughter uses both. It can certainly be done without the CD, but for us the CD cements learning, is FUN, and speeds up the process.

Re: The Reading Lesson: Just the book OR the combo??
I successfully taught my older son to read last year with just the book and no CD. But everyone seems to love the CD so I think I might get it for my ds in line.

We've been using TRL with book only, and when my son began to struggle with it about half-way or so in, I got lots of recommendations to buy the CD-lots of people seem to find it very helpful. It's hard to locate the CD only, so I'd definitely recommend starting with the combo pack!
