I also wouldn't necessarily worry about putting her in level 3. Like the other ladies mentioned, level 3 isn't hard. But level 4 is a jump and level 5 is even more so. You would do both the extensions and the history portion. But the part that you could omit (if you felt like it was just too much) is the story time box. The extensions are basically replacing that box with higher level books and with a bit more work.

I think there is a schedule in some of the new guides. However, we just do one lesson per day. I do math 5 days a week with the program. It works great for us to do it this way.

And I'm very excited for you to be retuning to HOD. We tried so many things prior to using it and that was probably good. That way I would know what I love and HOD is what I love! When I found HOD 4 years ago, it was the best year we ever had. Ever since then, we have stuck with it. My kids love it....
