I made a spreadsheet that lists every DITHOR book, sorted by level (3/4, 5/6, etc.). I don't have the grade equivalency for each book listed, but if the spreadsheet would be helpful, I'd be glad to share it with you.

Instead of using MOH1 or MOH2, HOD uses other (phenomenal) resources to cover the history up until the time period of MOH3. (MOH3 is not used in RTR until Unit 15.)

It’s on the website- if you are shopping for the packages it shows you where on the guide it is and it tells you how often. Visual History says 4 days and is highlighted on the music box.
